What Fertility Testing Should Actually Include (Beyond Basic Labs)

Many women struggling to conceive are told their fertility testing is “normal”, yet they’re still not getting answers.
They may have regular cycles. They may have already tried timed intercourse, supplements, IUI, or even IVF consultations. But despite doing “everything right,” something still feels unresolved.
This is often where functional fertility testing becomes important.
At Vital Fertility in Santa Cruz, we look beyond surface-level fertility markers to evaluate the underlying physiological patterns that may be impacting conception, implantation, hormone balance, and reproductive resilience.
Because fertility is not random biology. It’s responsive biology.
The Problem With Basic Fertility Testing...conventional fertility workups often focus on a narrow set of markers:
- AMH
- FSH
- Estradiol
- Progesterone
- Ultrasound findings
While these markers are needed and can be helpful, they rarely provide the full picture of what may be affecting fertility physiology.
Many women experiencing: delayed conception, unexplained infertility, recurrent miscarriage, failed IVF cycles
irregular cycles or hormone imbalances; have deeper physiological patterns that are either overlooked or considered “normal” despite being functionally suboptimal.
What Our Functional Fertility Testing Evaluates is, it is designed to assess how the body is actually functioning, not simply whether lab values fall inside a broad reference range.
At Vital Fertility, our fertility lab evaluations may include markers related to hormone Balance.
Hormones influence; ovulation, implantation, egg quality, cervical fluid, metabolism, sleep, mood, and cycle regularity.
We evaluate patterns involving:
- estrogen
- progesterone
- testosterone
- LH & FSH
- prolactin
- cortisol-related physiology
- methylation
- thyroid and prolactin
because hormone imbalances rarely happen in isolation.
Thyroid Physiology, even subtle thyroid dysfunction can affect:
- ovulation
- implantation
- pregnancy maintenance
- energy production
- metabolic resilience
Many women are told their thyroid is “fine” because their TSH is within normal range, despite symptoms or fertility challenges. To better understand thyroid physiology and reproductive impact functional fertility testing may include:
- TSH
- Free T3
- Free T4
- Reverse T3
- thyroid antibodies
Metabolic Health & Insulin Patterns: Blood sugar and insulin signaling play a major role in reproductive health.
Insulin resistance and metabolic dysfunction may contribute to:
- irregular cycles
- PCOS patterns
- inflammation
- ovulatory dysfunction
- egg quality concerns
Markers like; fasting insulin, hemoglobin A1c, C-peptide, triglycerides, ApoB, can provide insight into underlying metabolic stressors affecting fertility.
Chronic inflammation & Immune Stress may influence:
- implantation
- egg quality
- sperm quality
- ovarian function
- reproductive signaling
Markers such as the following can help identify hidden inflammatory burden or physiological stress:
- hs-CRP
- homocysteine
- CBC patterns
- nutrient status
Nutrient Status & Cellular Function, fertility is energy-intensive biology.
Suboptimal nutrient status may impact:
- ovulation
- hormone production
- methylation
- mitochondrial function
- reproductive resilience
To identify overlooked deficiencies that may be affecting reproductive health. Functional testing may evaluate:
- vitamin D
- magnesium
- iron markers
- anemia patterns
- protein status
Male Fertility Matters too, male factor infertility contributes to a significant percentage of fertility struggles (around 50%), yet it is often under-evaluated.
Functional Male fertility assessment may also include evaluation of:
- Male blood serum markers
- sperm quality
- motility
- morphology
- oxidative stress
- sperm DNA fragmentation
- toxin load
because conception is not solely dependent on female physiology.
Fertility Isn't about “Trying harder or consistency”...“just keep trying”, “everything looks normal”, “it’s probably stress”
“you can just start IVF”. Fertility challenges are often rooted in physiology that has not yet been fully evaluated, and examined. The goal is not simply to do more. The goal is to understand what your body may be communicating.
Our Approach at Vital Fertility
At Vital Fertility, we use strategic fertility testing and personalized interpretation to help identify your physiological patterns that may be impacting conception.
Our process is designed to help women and couples move from:
confusion → clarity
guesswork → strategy
symptom management → biological understanding
so next steps are grounded in biology, not guesswork.
This is why I created the Fertility Foundations Lab, this is designed to evaluate key biomarkers related to:
- hormones
- thyroid physiology
- metabolic health
- inflammation
- nutrient status
- reproductive resilience
to help uncover what may be disrupting fertility physiology beneath the surface.
